This charge includes basis service of funeral director and
staff, local removal of remains, embalming, shelterization
of remains, necessary authorizations and documents. This
charge does not include visitation, rites or ceremonies,

This charge includes transportation of the remains to disposition site
and basic services of Funeral Director and staff.
This charge does not include visitation, rites,
or ceremonies, out of county ground transportation,
or merchandise.

The charge for an immediate burial includes
removal of remains, care of unembalmed remains
and refrigeration. local transportation to the
cemetery, necessary authorizations. and basic
services of staff. This charge does not include
use of facilities and staff for any visitation
or ceremony, public or private, prior to burial.

| Terms of Payment |
| As the person(s) arranging
for this funeral, you have accepted
several obligations for final disposition
related expenses. At the conclusion
of making funeral arrangements, a
preliminary Statement of Funeral Goods
and Services will be completed and
presented to you for your acceptance.
Your signature on the statement will
indicate that you accept this obligation
personally and in any official capacity
you may have. Therefore, to avoid
any misunderstandings, Zimmer Funeral
Homes Inc. has adopted the following
terms for payment of funeral expense: |
| At the conclusion
of the arrangement conference or prior
to performing services, payment of an
amount equal to the initial cash advances
may be required. It is normally required
that most cash advances be given to
those parties rendering service on the
day of the funeral. A final statement
will be sent to you approximately 7 days
after the funeral service. The balance
will be due within 30 days from the date
of the arrangement conference. If full
payment of the funeral expenses is not
received within the 30 day timeline, a
late payment fee of 1.0% (12% per annum)
will be added to the unpaid balance. If
an estate MUST be set-up to pay for services
rendered, we reserve the right to add
our late payment fee after 30 days. Visa,
MasterCard, American Xpress Card are accepted,
and there will be a 2% merchant card handling
fee included. If you have any questions
regarding our terms, please ask. We will
try to accommodate your financial needs. |
